- QUESTION: My husband is a smoker for the past 30 years, and has never been
- able to stick to any program that could have helped him. Now he won't even
- consider getting the new patches that we have both read about, probably
- because nothing has ever worked for him. Among his many questions is one I
- would like the answer to as well. If nicotine is so bad for you, why use a
- patch that is loaded with it? Won't the patch do as much harm as the
- cigarettes? And if you get hooked on the patch, then what? Please try to
- answer these very important questions in your column. It is one thing my
- husband still respects.

- ANSWER: Your questions reflect many of the concerns contained in letters from
- other readers. But though the nicotine is the substance that causes the
- dependence that smokers experience, it is not the chemical that causes the
- most damage. Lung cancer is the result of the irritants contained in the
- tars, and many lung as well as heart problems are aggravated by the carbon
- monoxide in the smoke. There are probably more than 4000 chemicals in the
- tar, all potential causes for disease. Using the patch can still provide a
- nicotine "fix", but without all the other dangerous chemicals contained in
- cigarette smoke. And that dependence can be managed with much greater success
- than with cigarettes.
- The individual patches contain enough nicotine to reduce the craving for
- a smoke to manageable levels. That makes it easier to stop smoking completely
- from the very first day. A new patch is used each day, but after a period of
- time, a month or so, a patch with a smaller quantity of nicotine is
- prescribed. Two weeks later, the strength is cut again and a less potent
- patch is substituted. By reducing the daily intake slowly, the physical
- dependence on nicotine is overcome.
- While the patch takes care of nicotine dependence, some changes in life
- style help to combat the underlying stress which is so often the basis for
- smoking. An exercise program is often recommended to be used in association
- with the patch, and low calorie snacks and gum can be helpful items to replace
- the urge to put something in the mouth. Tell your husband to give this
- program a chance, it has been reported twice as successful as previously used
- methods.
